ONLINE NOMINATION FORM

NOMINATIONS FOR FUTURE INDUCTION CONSIDERATION MAY USE THIS FORM.

To be eligible for enshrinement in the Tracy Sports Hall of Fame, a nominee should have one of the first three requirements:

1. Having been born in the Tracy area
2. Gone to school in the Tracy area
3. Contributed to athletics in the Tracy area
4. Minimum thirty (30) years of age

If you would like to nominate an athlete, coach, athletic administrator, contributor, team, or a family for enshrinement in the Tracy Sports Hall of Fame, please use the online form below.  

The form starts with entering information about the person being nominated.  The last part of the form is information about you as the submitter so we can contact you.  Once completed, click on the Submit button at the bottom of the form.
